Referral Programs

MHRA has a few different referral programs available for its’ applicants.  Here is a list, including the qualifications and required documents.

Can be referred by MHRA Employee

  • Non-Elderly Disabled Voucher (NEDV)
    • Disabled Head of Household or Co- Head of Household
    • Between the ages of 18-62
    • Need an active MHRA application
    • Need social security letter, picture ID, social security card
      • There is also a packet that can be sent directly to doctors to verify disability
      • Homeless, at risk of homelessness and proof of homelessness
      • Disabled member of household (does not have to be
      • Head of Household or Co- Head of Household
      • but does need to over 18)
      • Between the ages of 18-62
      • Need an active MHRA application
      • Need social security letter, picture ID, social security card
        • There is also a packet that can be sent directly to doctors to verify disability

Must be referred by outside organization

VASH

  • A referral from the VA

FIT

  • A complete homeless referral
  • Must be from Families in Transition

Homeless

  • Referral from agency (Mental Health Center, Waypoint, NHES, The Way Home, YWCA, International Institute, Hospitals)
  • Must be homeless
  • Have an active service plan that addresses the cause of homelessness
